We have just recruited cohort 3. A new set of sites just starting on their AFN journey. I thought I would take this opportunity to share the feedback from the measurement questionnaire results that we collected at the launch event. We ask the questions to give us a head start when it comes to the measurement journey. We have 12 sites in cohort 3, but I only managed to get 11 questionnaires back, so the following is based on 11. Most sites in cohort 3 do not identify frailty at the front door, although 4 do, with one site (Lancs) having collected it systematically for over 4 years. Only 2 sites identify frailty 24 hours a day, 7 days a week. Blackpool are the only site in cohort 3 that have a computerised system for collecting frailty, although this is the plan for many of the sites. So a potential learning opportunity for us all there. Just over half of the new sites are planning on opening a frailty unit as a part of the AFN experience, this of course is not necessary to deliver great frailty care. Collection of data around CGA will be a focus for the year with 7 sites saying they collect data on CGA, but no sites collect any timings around when it starts or it has been finished.
From the launch event and the first couple of site visits, it looks like a fabulous cohort, with lots of sharing opportunities, see you at the next event.
