with the day starting at 10am and finishing by 4pm

 

Overview

This first event will give you and your team an opportunity to network with other sites plus an overview of what the AFN team have to offer.  Professor Simon Conroy will present the Acute Frailty Network Principles and Toolkit which provides a framework to guidance on how to implement better models of care that reduce hospital admissions and improve experience and outcomes for older people.  There will be case study presentations, an introduction to measuring your impact and how working with the network will help the spread of learning across participating sites.
